Patient Outcome Reporting for Timely Assessments of Life with Post-Traumatic Stress Disorder

Post-traumatic stress disorder (PTSD) is a common condition with long-term effects on health and well-being. People residing in environments impacted by high levels of violence may develop PTSD symptoms, even without direct personal experiences. Many are not identified due to reluctance to disclose symptoms, time limitations, or missed opportunities during care. Currently, it is not a part of routine primary care practice to screen for PTSD, except with the Veterans Affairs.  We aim to examine for the role of PTSD screening and linkage to integrated behavioral health care within primary care in partnership with Chicago Family Health Center.

People who screen positive for PTSD will be offered support through a team-based care model that includes behavioral health and primary care staff working together. The goal is to identify PTSD earlier and understand how this changes outcomes for patients and the healthcare system.
